While the majority of children’s hospitals are screening for social drivers, they have not yet developed standardized questions or an organization-wide process.
Phoenix Children's Care Network (PCCN) successfully implemented social drivers of health (SDoH) processes within its primary care network, overcoming common challenges. In 2022, PCCN implemented a systemwide social driver screening tool designed to ensure screenings are short, simple, effective, and can easily be integrated into the clinical workflow.
Driven by a continuous quality improvement methodology, explore how PCCN:
- Created a SDoH network guide with best practices.
- Developed a screener and screening process.
- Integrated care coordination mechanisms.
- Implemented a closed-loop communication process.
- Formed internal support mechanisms through optimized processes, resource allocation, and data analysis.
PCCN will also review the critical need for appropriate data support and robust data dashboarding to monitor and enhance program effectiveness.
